By letting patients take and send their medical readings virtually from anywhere at any time, you are enabling your patients to maintain more active and familiar lifestyles. They are able to remain in control of their medical conditions, rather than their conditions controlling them.  

Wireless remote monitoring also enables you to get the data you need, when you need it.  As a result, patients can be provided with the most appropriate levels of education, treatment and intervention for their conditions.  By detecting changes in medical conditions, even before patients begin presenting symptoms, therapies can be adjusted and costly inpatient admissions avoided.

MedApps provides you with these types of capabilities through a dynamic, three-step process:

Costs are typically based on a per-patient/month basis, which is inclusive of unlimited access to monitoring software, remote servers, as well as the unit(s) being used by the patient.  It also includes all data transmission costs.

 

Recording and Transmitting Patient Monitor Readings

Enabling remote patients to provide health data requires the patient to purchase, or be provided with, the appropriate health monitoring device, such as a glucometer, and the MedApps HealthPAL.

When a patient uses the glucometer (or other type of health monitor) to obtain a reading, the HealthPAL automatically obtains the information and transmits it, using cellular-based technology, to a secure server for monitoring and viewing by your nurse call center.

The HealthPAL represents significant breakthroughs for DMOs that recognize the value of remote patient monitoring.

-          Mobility  The basic underpinning of MedApps technology is to enable wireless transmission of data from virtually anywhere a patient may be – whether they are at home, at work or on the go.  This enables patients to maintain active as possible lifestyles – no longer tethered by monitoring equipment that has to be connected to the patient’s home telephone line. With fewer restrictions impacting a patient’s lifestyle, the easier it is for them to be compliant.

-          Simplicity  MedApps ensures nothing gets in between the data and your call center nurses.  Once a reading is taken, the HealthPAL records it and transmits it without the patient having to turn anything on or off, connect any cables, push any buttons, or navigate any on-screen commands.  As a result, there is virtually no assistance required to set a patient up or technical support needed down the road.

-          Affordability  The HealthPAL itself costs no more than most portable consumer electronic devices.  Much more significantly, it enables you and your patients to use “off-the-shelf” health monitoring equipment and supplies that are competitively priced and readily available on-line or at virtually any drug store, grocery store, discount store or similar outlet. It is possible that for some patients, they would not have to even make a change from the monitor they are using today.

Receiving, Storing and Analyzing Patient-Sent Data

This part of the process is completely transparent to both your organization and the patient.

The HealthPAL automatically sends patient monitor readings to a secure central server where it is stored and on which all data analytics are performed.

The server runs the MedApps HealthCOM software application, which your nurses can access through their web browser.

Using a centralized, web-accessed, secure server provides your organization with numerous benefits, including:

  • Complete system implementation without any impact on your existing IT infrastructure or additional costs for new hardware or system upgrades
  • No internal technical support, maintenance or software management overhead required  
  • Easy access by nurses, whether they are in the field, at the call center or working from home

Viewing, Monitoring and Acting Upon Patient-Sent Data

By accessing the MedApps HealthCOM server, nurses can utilize their web browsers to actively monitor all of their patients as well as run important and often revealing analytics on an individual’s information.  The data can also be used to populate your back office system.

Individualized thresholds and timetables can be established for patients that will automatically trigger alerts when patient readings fall outside their parameters – or if data is not received at all.  When this occurs, not only is the nurse alerted, but automated interactive voice response (IVR) calls to the patient can be triggered to serve as a reminder, alert or facilitate connecting the patient with a nurse on the spot!

All readings and events can be reviewed, notated and used to create a wealth of reports and graphic trending snapshots – all of which can be exported in various file formats.

Patients are easily added to the system and can be assigned, reassigned and managed just as easily.  Your nurses can maintain significant levels of patient information on the system so they have everything they need at a glance to provide quality care and excellent customer service.
